IMPORTANT INSTRUCTIONS FOR ACCESSING THE FORMS
When you click on one of the above links, most browsers will open the form you’ve chosen in a new window. When this happens, follow “IMPORTANT INSTRUCTIONS FOR FILLING OUT THE FORMS.”
If the form doesn’t open in a new window, you can:
- Right-click on the link and choose “Save link to…” or “Download file to…” from the pop-up menu.
- Navigate to a directory of your choice on your hard drive and save the form (or save it to your Desktop).
- Open the form using Adobe Acrobat Reader. (If you don’t have this app, you can download it here.)
- Follow the instructions in Option 1 or Option 2 below.

FOR FILLING OUT THE FORMS
You have two options for filling out these forms.
OPTION 1: Fill out the form manually
- Print the blank form from your browser. Most browsers will display a Print icon, or you can choose Print from the File menu.
- NOTE: If you’re unable to print the form from your browser, follow the instructions in the previous section and download the form to your hard drive, then open it and print it.
- Complete the form (use a pen). Be sure to fill out all applicable fields.
- Mail the form to:

On the ATTN: line, circle the appropriate department depending on which form you’re sending in.
You may also bring your form to the court in person during regular hours. Emailed forms will not be accepted.
OPTION 2: Fill out the form electronically
- Download the blank form to a directory of your choice on your hard drive (or to your Desktop). Most browsers will display a Download icon, or you can choose “Save As…” from the File menu.
- IMPORTANT: Always download the form to your computer’s hard drive first. Do not attempt to fill the form out electronically from within your web browser.
- Minimize your browser, then locate the form on your hard drive and open it using Adobe Acrobat Reader. (If you don’t have this app, you can download it here.)
- Complete the form electronically. Be sure to fill out all applicable fields.
- NOTE: You can tab between most fillable fields on the form. It is usually not necessary to click from field to field with your mouse, although you may need to click to go to a field on the next page of a multi-page form.
- NOTE: For multiple-line text fields, you must tab to or click in each new line. Your cursor will not automatically jump to the next line in a multiple-line text field.
- Print your completed form and mail it to:
